Care Management

 

If arranging care has become too difficult to manage, and you need someone to help coordinate a loved one’s medical (and other) affairs, you may benefit from the help of a professional Care Manager. Our knowledgeable, experienced staff can offer as much – or as little – assistance as desired. 

 

We can provide any or all of the following fee-based services:

• Bill Paying Services

• Caregiver coordination and monitoring

• Counseling

• Assessment for depression

• Guiding through the Medicare/Medicaid process

• Transportation and attending to medical appointments

• Coordinating community services and programs

 

The first step is conducting a home assessment. We can then create a care plan with the goals of safety, well being and stability. Depending on the case, we follow up with weekly or monthly monitoring.
